In this lecture, Dr Frederick Weltdiscusses haemodynamic guidance in cardiogenic shock, with a focus on early intervention, therapeutic goals, and the importance of an exit strategy. He reviews left and right ventricular failure, device options including balloon pump, Impella, TandemHeart, and VA-ECMO, as well as the role of pulmonary artery catheterisation and haemodynamic monitoring.
Drawing on recent studies, registry data, and practical experience, Dr Welt highlights how SCAI shock staging, institutional capabilities, and patient phenotype influence decision-making. He also addresses the limitations of current evidence, the need for escalation strategies, and the real-world challenges of balancing device benefits with complications such as bleeding or limb ischaemia.
