In this first lecture of the Complex Cases, Dr Frederick Welt explores how cardiogenic shock is defined, staged and managed in modern practice. He outlines the limitations of older definitions and introduces the SCAI shock classification (Stages A–E), a practical bedside tool that improves prognostic accuracy, guides treatment decisions and supports clearer communication between providers.
Dr Welt then examines the rise of multidisciplinary shock teams, showing how structured pathways, rapid identification and coordinated interventions can improve patient outcomes. Drawing on institutional data, he highlights real-world survival benefits and discusses the role of therapies such as inotropes, Impella, balloon pumps, and VA ECMO. This session provides a clear framework for recognising, staging and treating cardiogenic shock.
