    Cardiac MRI is firmly established as the gold standard diagnostic imaging investigation in heart failure, due to its capability to accurately assess cardiac function and anatomy with excellent reproducibility and its novel ability to non-invasively identify specific aetiological processes.

    In the past 2 decades there has been a rapid evolution of contrast-enhanced techniques and subsequent increases in their utilisation in clinical practice and heart failure research. This has led to a large prognostic evidence base. Cardiac MRI is now recommended as the first-line imaging investigation in the current National Institute for Health and Care Excellence, European and North American heart failure guidelines for cases where transthoracic echocardiographic windows are poor or there is diagnostic uncertainly regarding aetiology.
